Wound Care
Professional management of acute and chronic wounds to promote healing and prevent infection.
Medication Management
Assistance with administering and managing medications to ensure safety and adherence.
Post-Hospital Support
Tailored care to assist your recovery after a stay in hospital or surgery.
Palliative Care
Compassionate, dignified end-of-life care focused on comfort and support for both the veteran and their family.
Personal Care
Assistance with daily tasks such as showering, dressing, and hygiene when there is a clinical requirement.
How to Get Started
Accessing DVA Community Nursing is a simple process:
Visit Your GP
Request a referral for “DVA Community Nursing”. You can specifically mention Carezone Nursing Solutions as your chosen provider.
Referral Received
Once we receive your referral from your GP or hospital discharge planner, our team will contact you.
Comprehensive Assessment
A Registered Nurse will visit you at home to conduct a full assessment and develop a personalized care plan tailored to your needs.
Commence Care
Your dedicated nursing and support team will begin delivering services in your home.
Carezone Nursing Solutions: DVA Health Cards (Gold and White) are accepted as payment upon GP referral.
